Check to make sure everything you have written is correct because our art deparment uses the capitalization you enter.
Customized pads are manufactured from the information you have entered using this form.
Proofreading is your responsibility for any errors or blank fields left.

Once you have finished entering the information into the form, please click on the Submit Pad button.
The information you have entered will then display on top of the form. The form will remain incase you wish to change the fields you have entered.
Then you can upload your companys logo in vector artwork. For help on submitting your artwork please take a look at our Artwork Submission page.
If you wish to upload the artwork here, please upload the file below or you can send your artwork by email (sales@usapad.com) if the file is too big to be uploaded here.
Once you have read the information with the proof, agreed to the information, then add your finalized product to the cart.

You may upload jpegs, gifs, pdfs, psd, eps, and ai.
The upload size limit is 2MB.
Any other file will not upload.
If you try to save an image that’s
not a jpeg, gif, pdf, psd, eps or ai, the result will be
an error displayed or the same the same page will
be displayed the same way now, with no message
saying your image was saved.
